What Is DHT?
DHT, or dihydrotestosterone, is a hormone made when an enzyme called 5-alpha reductase converts testosterone into DHT. It happens throughout the body, not just the scalp, and it is not inherently harmful. The problem is specific to hair follicles that carry a genetic sensitivity to it.
Why Some Follicles Shrink and Others Don't
Hair follicles on the scalp are not all the same. Follicles at the crown and hairline often carry more androgen receptors than follicles at the back and sides of the head. This is why pattern hair loss shows up in a predictable shape instead of affecting the whole scalp evenly.

The telogen phase lengthens. Telogen is the resting phase before a hair falls out and a new one begins. DHT-affected follicles spend more time here, which means more visible shedding and slower regrowth between cycles.
The follicle miniaturizes. With each shortened cycle, the follicle produces a slightly thinner, shorter hair than the one before. Over repeated cycles, follicles shrink from producing thick terminal hair to producing fine, short vellus-like hair. Eventually some follicles stop producing visible hair at all.
This is a gradual process, which is why DHT-related thinning shows up over months and years rather than all at once.
